Kindergarten Application Packet
Thank you for your interest in our school. We are happy to provide our application packet on-line for download. If you wish, you can type your information into our PDF and print your application. However, at this time, you cannot submit your application on-line.
The following Kindergarten Application PDF contains all of the application which must be returned to our school by February 13, 2013. In our Kindergarten Application PDF, please find the following information:

- School Recommendation Form
- Authorization for Release of Student Records
- Testing Schedule
Please give the School Recommendation Form to your child's current teacher. The teacher must send the School Recommendation Form to the school. We cannot accept an evaluation from a parent. The Teacher Evaluation is due on February 15, 2013. Our school address is below:

Please fill out the application form, and return with:
- A non-refundable $75.00 application fee. Please do not send cash.
- A copy of your child's baptismal certificate. Please do not send the original document.
Children must be 5 years old by October 1 in order to apply for Kindergarten. Also, if your child was born between October 1 and December 1 we ask that you take into account that our program is a full-day Kindergarten program which requires a level of maturity which may not be necessary for other Kindergarten programs.
Once we receive your application, you will be mailed a letter or called via the telephone to set a testing date. If more than one child is applying, you must complete an application form for each child.
